Kaiser Permanente (KP) was founded in 1945 at the height of the Great Depression (Kaiser Permanente, 2017a). Since their humble beginnings KP has strived to provide the best care possible for their patients and improve on health for their members. Kaiser has grown from a small hospital, to a large foundation by making appropriate changes and using strategic planning to ensure members receive the health care they need in terms of quality and safety. One method of strategic planning KP uses is appreciative inquiry, a method and philosophy that identifies when the facility functions at its best and builds from those situations to create a vision of the future and work towards that vision (McNamara, n.d). Communication in the operating room is very important. If surgeons and nurses are not communicating effectively it can directly affect the quality of patient care and safety. In 1999, the Institute of Medicine (IOM) issued a report, To Err is Human: Building a Safer Health System, which estimated the fifth leading cause of death in hospitals in the United States was due to health care errors (Mason, Gardner, Outlaw, Freida, 2016). To help reduce these errors, effective communication needs to be exercised throughout health care. Nurses and physicians need to express themselves in a clear and precise manner, their message should rely on verification and collaborative problem solving. They need to displaying a calm and supportive demeanor under stress, maintenance of mutual respect, and authentic understanding of the unique role (Robinson, Gorman, Slimmer, Yudkowsky, 2010). Communication in nursing is known for its life saving success as well as its greatest flaw in poor patient outcomes. There is always room for improvement and when communication is carried out efficiently, healthcare professionals have reaped the benefits. However, there have been many instances in which nurses have had to learn the hard way of how detrimental communication can be to patient safety. Through research and reviews of literature, the topic of patient safety related to handoff communication among units is analyzed.
As a leader, you are tasked with implementing new changes and leading your staff to accept and implement the changes. You must be able to present the idea, involve your staff in planning and trust them to come up with ideas to make the change happen. Leaders should motivate staff and know how to handle resistance (Huber, 2014). How you interpret change, whether you are accepting to it or refuting it, is how you will transpire that change in your staff. Despite this, there are certain routine activities which I had the opportunity to observe through my shadowing experience in the Emergency and Operating rooms at California Hospital Medical Center, Los Angeles. According to Davis (2014), “principle categories of evaluation include teamwork, communication, mutual performance monitoring, maintenance of standards and guidelines, task management, procedural skill, and equipment management.” This statement was pulled from an article about CCATT, critical care air transport team, missions in the Air Force, and focused on the importance of communicating within your care team. Davis (2014), showed important statistical information that expresses something called “task saturation” which is best explained as the amount of activity happening in the critical treatment times.

People in any community are exposed to preventable medical errors; a rate that ranges from 20-42% (3). The undue consequences of such medical errors require studies focusing on patient safety, generally concentrating on hospital environments (4,5).
