Personal Narrative: Duty Log

On 10/22/2015 SO EMT Perez was dispatched to FC 418 regaurding a fallen female resident. SO EMT Perez knocked and announce his presence at the door and was greeted by the spouse of the resident who invited him in. The fallen resident in question was on the kitchen floor sitting upright, a Mrs. Barbara Murtishaw. Mrs. Barbara Murtishaw had fallen in her kitchen and needed a lift assist because she could not get up on her own strength. Mrs. Barbara Murtishaw stated that she was not in any pain and only wated to be lifted upright.

On the indicated date and time I Deputy Garcia #618055 was assigned to staff station 620 at North County Correctional Facility (NCCF) when I heard a loud thud which is consistent with that of fighting. I saw Inmate Lopes, Juan #3456712 (later identified as suspect/victim 1 (S/V)) and Inmate Henry, Chuck #2345678 (late identified as suspect/victim 2 (S/V)) on the ground by the day room punching each other on their face and torso. I put a radio broadcast of a minor 415 inmates in dorm 623 with my county issued radio (minor 415 is a common radio code in our facility which indicates a fight is in progress or just occurred). I also requested additional personnel, Sergeant and Senior to my location. I ordered S/V1 Lopes and S/V2 Henry to stop fighting and they complied.

SGT. Barrett and I contacted a suspicious vehicle in the parking lot that was parked in an unlit area at approximately 2300 hours. Once outside of our vehicle I started flanking toward the right side of the white Nissan Maxima, as the windows were darked out. SGT. Barrett went to the driver side of the vehicle, where the door was ajar, with a male sitting in the driver's seat with his feet planted on the ground I heard what sounded like a dense metal object fall onto the pavement from the driver's side of the vehicle.

On 07-24-2016, Sergeant Koch and I responded to 7520 December Drive, reference a burglary that occurred on 06-24-2016. I have been trying to make contact with the suspect, Tyler Gray, for the last two weeks with negative results. I previously responded to Gray 's residence and made contact with Gray 's father, William Gray. I have left William my business card with instructions to contact with me. Upon arrival, Sergeant Koch and I made contact with, William Gray.

Last year when I was in 8th grade I joined the track and field team. My main event that I did was the 100 meter dash I got an amazing time of 13.7 seconds, apparently I ended up qualifying for states in that race. I also made a solar system poster that took a lot of work but I got an A+,on it. The project was an extra credit project.

Staff intervened and shut chis ' door informing him to lock his door. CCIB Intake received a SOC 341 from reporting party Al Vanderhoeven who stated on 3/5/17 resident Paige Katler was arguing with resident

CM completed an overnight pass request for client R- Client presented well-groomed and with good hygiene. The client appeared calm and made good eye contact, was cooperative, and open for discussion. His affect was appropriate and his mood appeared euthymic. Housing: Client has been assigned to a LAMP VASH worker, Laurie Garza.

On 10/26/16 CM went to unit #222 due a complaint that water was coming from that unit causing some damages to the ceiling in unit #122, situation that has been happing before and was fixed by the maintenance personal. CM ask Ms. Footman was happen she say that anything was fine in her unit, however based on CM observation , the bathroom tub was wet , signs that someone recently took a shower but the floor was dry. Ms. Footman denied having any flood in the unit at that moment. On 11/27/16 CM went to conduct the Safe Sleep inspection with the DHS Compliance Analyst Jeanette Chirico, while in the second floor we went to unit #222 assigned to Ms. Tiana Footman and her three children.

On Sunday, 01/24/2016, at approximately 20:18 hours, Jackson County dispatch received a 9-1-1 call from a Brittany R. Smith (F/W, DOB: 12/26/1991) reporting a domestic disturbance at 209 N. Walnut St. Desoto, IL 62924. Sergeant Ken Lindsey #2406 and Deputy Brandon Crain #3415 were dispatched. I, Deputy Stacy Stark #1815, responded to the scene as well. I arrived on scene at 20:21 hours.

“Log number 17: We have been lost for 10 weeks in this tundra, if anyone is hearing this then we are alive and I published this with my other log dates,” I chattered, while writing. “ What are you talking about?” questioned my pal Tom, but he goes by SANS. I replied with, “ I’m writing my 17th log date, you know I talk to myself when I write.” I have made 16 other log entries and he still questions me about this. But my cousin Sunnshine, my friend SANS (Tom), and I have been stuck in this stupid tundra in the middle of Greenland for 10 weeks, with limited food, freezing temperatures, and the University (the building we work for) completely forgot about us, go figure.

In this day and age, being on time for school is a thing of the past. But to be honest, the night before I was having the time of my life going through all 13 seasons of Grey 's Anatomy. In any way, shape, or form did I know that I had a stack load of homework to get done for the next day. But lets cut to the chase, it was a 4 day weekend which meant catching up on both sleep and netflix. Even though time was of the essence, I procrastinated the whole weekend to get the homework done.

My Worst Job When I was sixteen, one of my middle school friends named Jessica was a shift leader at dunkin she wanted me to work with her for the summer to have some extra money. It seemed like a lot of fun for the first week or so, but it turned out to be the worst job of my life. My job was the front cash register. This was fun I actually enjoyed it

Eight years ago I walked into a Navy recruiter's office and said, " If I join today, when is the soonest I can go to boot camp? " I did not know what to expect. However I was sure of one thing, and that was that I wanted to join the United States Navy. As those eight years ensued, the Navy began to mold me physically and mentally. Some of the changes I underwent were positive, and others were rather uncouth.

Overworked. That’s the closest word that I could use to describe this week. I feel like this journal is going to be about me just bickering, yet there is some stuff you might want to read about. First of all, I have been sleeping three hours this week because of upcoming midterms, quizzes, and assignments due. I am sleep deprived and mentally drained and as my second year in college I have never had my life drained out of my body like a passing shadow.

A sense of accomplishment is invaluable to a person. Not only does a sense of accomplishment build confidence and faith in oneself, but it also allows one to reflect on how wonderful the journey to the accomplishment was, and how every little struggle and triumph was worth it. In the middle of summer, where time seems endless and the stress of the previous school year has been shed by students, I never expected to find out that I scored a five on both of the advanced placement exams I took. Nor did I have one-hundred percent confidence the goals we set as section leaders of the marching band would actually be met. Yet to my surprise, I had the good fortune of accomplishing challenging things in both aspects of my life.
