Mr. S is a 60 year old male who presented to the Medical Day Center at the JGH on October 19th 2015. He had an appointment with his doctor so that the burn on his right foot could be verified. The previous week on Monday, he had burned his foot by dropping oil on it, and presented himself to the ER. On Tuesday he went to the Plastic Surgery department to get his foot examined. On Thursday, an infection occurred. An antibiotic IV treatment was initiated Friday, with the CLSC nurse coming to his home on Saturday and Sunday to change his dressing. Mr. S ambulates with a wheelchair, but he is able to transfer from wheelchair to chair independently. The patient is divorced and lives alone. The patient is pleasant and has a very nice demeanor. The patient is clean and well-dressed, and still takes care of his hygiene. The patient denied pain when the wound was examined. Deficits: The patient does not have a wide support system. The patient lives alone for the most part. He is in a wheelchair. He stated that he feels "stuck" because of his situation, and being unable to work and do his ADLs brings him great dissatisfaction. He was reluctant to go to his appointment at plastic surgery the following day. Risks: Living alone can be a risk as there may be some things that he is unable to do like when he could ambulate. Depending on the configuration of his home, there may be instances when he has to get out from the wheelchair, and he could have a fall. Furthermore, living alone also carries the risk of social isolation due to him not being able to go outside as often as before or continuing his hobbies normally. All of these could contribute to negative feelings and decreased quality of life. Also, nonadherence to treatment plan, for whatever reason, is likely to result in worsening of the situation. DOI: 4/16/2012. Patient is a 29-year-old male technician who sustained injury when he was 25-feet up on a ladder when the ladder slid and he fell onto the pavement. He had an open reduction internal fixation (ORIF x 2) for a compound tibia fibula fracture and had hardware removal in 4/25/2013. MRI of the lumbar spine performed on 3/24/2016 revealed L5-S1 small right paracentral disc protrusion without significant spinal canal stenosis or neuroforaminal narrowing.
A patient is admitted to Nightingale Community Hospital to the surgical unit following an infection to a post-op wound. There were several deficiencies found on the patient’s tracer audit once the patient was admitted to the hospital. One deficiency that was found was that the patient was given medication related to pain and the patient was not reassessed properly per Joint Commission Standards (JC). The deficiency found is within the pain assessment policy of the hospital.
