The “Evidence-Based Practice: Step by Step” articles, by the American Journal of Nursing explains how quality of care is improve though evidence based practice. By preforming the step method of approach, nurses can gain the ability to conduct an effective search for data to improve patient conditions. In order to apply the steps of evidence based practice, the article follows a case scenario of a nurse named Rebecca, though some of the following steps. The articles stated that there are seven steps in the process, beginning with step zero, which states that the nurse must have a “spirit of inquiry” in order to determine that there is a need for an intervening question. There are multiple factors in the role of finding the best evidence, such as determining the level of evidence, selecting the most relevant databases to conduct the search, and choosing keywords from the PICOT question. The article stated that Rebecca searched for studies that could provide her with strong evidence that could be used to support her possible practice change. She searched within databases that would give her relevant information towards her topic such as CDSR and DARE. To find the relevant data she used keywords within the PICOT question, then she applied search strategies such as using “and” and selecting defining parameters. For step three and four, the nurse must critically appraise the evidence and integrate the evidence with clinical expertise and patient preference and values. The nurse must question the results of the study and determine that they are relevant in the search. After, the nurse must come to a conclusion about the studies and decide whether they have the same results or support her change. a. This portfolio shows the educational journey through the BSN program at Western Governors University(WGU). The curriculum at WGU was very challenging, nonetheless it prepared me to become a safe and effective nurse. The BSN curriculum provided avenues to keep me up to date with safe practices and learn how to master therapeutic communication. WGU also helped me sharpen my critical thinking skills in order to make decisions quickly and provide safe and effective care to patients. Deb Kanya Initial Post Polit & Beck, (2012) describe Evidence-Based Practice (EBP) as an integration of clinical expertise, patient values, and the best research evidence. One of the more challenging aspects of EBP is the actual research on a particular topic. The fact is there is a multitude of journals and reviews etc. on any given subject; for this reason it is imperative that one knows how to conduct a proper search for pertinent information. Due to the complexity of literature searches and the amount of information available it is prudent to follow a guide while doing research.

According to Hinkle and Cheever (2014), Evidence-based practice is not about emerging new knowledge or confirming existing knowledge. It is about interpreting the evidence and applying it to clinical decision-making. And clinical opinion validates existing knowledge based on a theory. Furthermore, Evidence-based practice is a practice that is supported by the reaches that considers the health care setting, patients, and clinical judgment (Hinkle and Cheever, 7) This week I was able to practice in relaxation techniques with my patient.
Evidence Based Intervention Case Information Mary has been diagnosed with Alzheimer’s disease at a moderate level. She also has other health issues including high blood pressure, diabetes and depression. Her husband, John has these additional health issues as well and is currently Mary’s caregiver. They receive home health services as well. John understands that Mary will need to go to a nursing home at some point, but would like to keep her at home for as long as possible.
Examples of such research are clinical trials, experiments conducted in the laboratory, and controlled random trials. The second component is clinical expertise, the observation and exposure gained over time. An example of clinical expertise is inductive reasoning. The third and final component of evidence-based practice is patient benefits. This is the “unique preferences, concerns, expectations, financial resources, and social supports” each patient brings to the clinical consultation (Greiner & Knebel, 2003, p. 57).
When implementing evidenced-based practice, there are five major steps to its development: ask, acquire, appraise, apply and assess. Research involves gathering data, comparing, and analyzing it to produce information that is viable to a particular topic being evaluated. Critical appraisal is one of the most important components to evidence-based practice. According to the Center for Evidence-Based Medicine (n.d.), critical appraisal is the process of carefully and thoroughly evaluating research. The reason behind critical appraisal of research is to identify the level of evidence, how well it was conducted and evaluate it for relevance to one’s practice (Fineout-Overholt, Melnyk, Stillwell & Williamson, 2010).
Evidence based practice (EBP) is to demonstrate the best practice, which has been supported, with a clear rationale to back it up, while acknowledging the patient/clients best interest. Evidence based practice is the act of incorporating clinical expertise, best research evidence and patient values and preferences in delivering care. This system, as opposed to previous methods that used the same standard of care for each patient, evaluates treatment plans based on research and the practioners own experiences. The usual workup of this type of practice is to ask a series of “why” questions and meticulously observe patient patterns to paint a better picture of the environmental factors surrounding the patient’s condition. 1. CONCEPT 1-EVIDENCE-BASED PRACTICE IN NURSING This concept was deducted from module 2 with the topic “Concept of Evidence-based practice”. Evidenced-based nursing is a way of making decision and providing nursing care that is based on clinical issues and combine it with the most current, relevant research that is available on that issue. Evidence based nursing utilize the most up to date method of providing care, which have been proven via assessing high quality studies and statistically with significant research findings.
In the clinical setting, the nurses’ expertise can be used to make clinical assessments and recommendations for routine care at a lower cost than a physician visit. This would increase the number of patients seen and increase the quality of the care provided. The advance practice nurse would be able to hone in on preventative measures and increase patient education. The advance practice nurse can also make recommendations about practice changes needed to facilitate better health outcomes through the use of evidence-based practice. Nursing leaders are aware of how important nursing science is to provide needed evidence to transform practice, even though, finding the time and resources to support any research activity is often challenging (Stone, 2017).

Giving care to a patient is not a straightforward process because a patient is made up of advanced systems. Symptoms and the severity of a disease process are dependent on a particular patient, and it may not always be uniform from patient to patient. Because of this, nurses must be able to use their knowledge appropriately to help a patient. Nurses use techniques, such as Evidence Based Practice, in order to integrate new and advanced knowledge into their patient care (Canada, 2016). By exercising evidence based practice, nurses effectively seek knowledge, take experience from past situations, and apply this intelligence to best give patient care (Canada, 2016).
