The implementation and evaluation stages of the nursing process are important in delivering appropriate care to a patient. Implementation involves the actions of the nurse like recording, delegating, it can also be assertive, rehabilitative, supportive, preventive, educative and observational activities. Evaluation, on the other hand focuses on the outcomes of the implemented care, it is ongoing, as it appraises the extent set goals and interventions were achieved and provides the basis for reassessment (Wrycraft 2015: 16). The implementation and evaluation of care given to a patient will need good communication and collaborative partnership between the nurse, patient, and other professionals in the team.

According to the documentary, Ride the Tiger: A Guide Through the Bipolar Brain, despite the staggering number of people diagnosed with bipolar disorder, there is limited information available on how it appears in the brain. The documentary explored the tentative location of the biological breakdowns within the brain, with hopes to prevent the genetic vulnerability from developing the disorder, and rewire the brain to a recovered state for those already diagnosed with the disorder. The documentary suggested that there is not one road map which will guide people back to mental health wellness, because one problem is not presented, and there is not one solution. The video “Ride the Tiger: A guide through the Bipolar Brain” introduced and explored new insights in brain science that are being used to create new and more effective treatments for Bipolar and depression. It is important to understand about how the biological components of these disorders affect our patents and how effective medication can be. I can use this information to guide clinical decisions when working with clients who suffer from Bipolar or Depression. Initially, this video gave me a greater understanding about how much biology affects these disorders and the importance of incorporating referrals to a psychiatrist is in our work. Using medication to address these disorders can be a life and death issue because of the prevalence of suicide risk associated with them.
The details of the study included literature review, purpose, method, data analysis, results, discussion, limitations, nursing implications, recommendations for future research and conclusion. The results were clearly presented. Study limitations were identified. The conclusion was based on study results. This review was of good quality.

Evidence and Evaluation in Bedside Reporting Bedside reporting assist nurses with a chance to improve patient safety and increase patient collaboration in the arrangement of care. There is also less care correlated to inaccurate or deficiency of information because the report process includes actual patient apparition. Increased staff approval with bedside reporting supports teamwork and supports accountability. By associating bedside reporting there is an optimistic impact on the patient and their relatives.
An individual with bipolar disorder may experience a euphoric episode of intense energy and happiness. Then, without warning, they drop into a depressive state where they find little interest in anything. Anyone is at risk for developing bipolar disorder, but studies have shown that a person's background, trauma, and fame can significantly impact mental illness. The International Journal of Bipolar Disorders states, “There are consistent indications that childhood traumatic events are associated with various severe clinical characteristics of BD, including an earlier onset of the illness,”(“Role of Trauma in BD”).
Evaluation is done in public health to determine the progress of health program objectives and goals. Public health evaluations are important information that serves as a database in nursing diagnoses and also community nursing diagnoses from the assessment data that was analyzed, for any program in public health to be relevant; health program must be evaluated regularly. Evaluation is crucial in public health nursing practice as evaluation finding and existing program can be terminated unless evaluation evidence
As assessment is integral to the nursing process it is also incorporated into nursing models. Assessment is necessary during all nursing activities e.g. assisting an individual with their hygiene needs, taking observations or during repositioning/manual handling techniques. Orem’s model is a particularly effective tool in carrying out assessment as it has a practical approach in identifying patients’ needs by encompassing their universal, developmental and health deviation self-care deficits. ‘’Having a conceptual nursing model to practice may enable nurses to gather a detailed database that identifies actual and potential healthcare problems’’ (Capers, 1986). The grid, checklist format is a simple, fast and straightforward assessment guide and can be very useful in practice.
1. CONCEPT 1-EVIDENCE-BASED PRACTICE IN NURSING This concept was deducted from module 2 with the topic “Concept of Evidence-based practice”. Evidenced-based nursing is a way of making decision and providing nursing care that is based on clinical issues and combine it with the most current, relevant research that is available on that issue. Giving care to a patient is not a straightforward process because a patient is made up of advanced systems. Symptoms and the severity of a disease process are dependent on a particular patient, and it may not always be uniform from patient to patient. Because of this, nurses must be able to use their knowledge appropriately to help a patient. Nurses use techniques, such as Evidence Based Practice, in order to integrate new and advanced knowledge into their patient care (Canada, 2016).
