On 10/22/2015 SO EMT Perez was dispatched to FC 418 regaurding a fallen female resident. SO EMT Perez knocked and announce his presence at the door and was greeted by the spouse of the resident who invited him in. The fallen resident in question was on the kitchen floor sitting upright, a Mrs. Barbara Murtishaw. Mrs. Barbara Murtishaw had fallen in her kitchen and needed a lift assist because she could not get up on her own strength. Mrs. Barbara Murtishaw stated that she was not in any pain and only wated to be lifted upright.

On 1/17/2016 SO EMT Perez was dispatched to HG-407. SO EMT Perez knocked and announced his presence at the door. SO EMT Perez was greeted at the door by the resident. The Resident, a Mrs. Elsie Cooperman answered the door and was activly bleeding from her face as she was trying to explain what had happened. SO EMT Perez immediatly began to controll the active bleeding and had Mrs. Elsie Cooperman sit down in a nearby chair while he tried to also calm the resident down so he could get some information about what happened.

CCIB received an Incident Report from the LTCO Maged Mansour (213) 351-6433 regarding resident Maria Alcantara. The SOC 341 states that Maria disclosed to facility staff and her Day Program staff that residential facility staff member Gloria G hit her in her back. Once notified by of incident the facility staff notified the administrator Kendal Woods (323) 208-0133. According to the RP the incident is under investigation. The suspected abuser would be suspended pending

Ps, Elizabeth Gibson and Gerding Speare, allege excessive force and false arrest. Apartment # 510: P Gibson alleges that was on the toilet when MOS break into the bathroom with their guns drawn and pulled her off the toilet. P Gibson states that she was not allowed to get dressed and was dragged into the living room. P Gibson claims she was handcuffed tightly. Apartment # 406: P Speare alleges that she was in her bed when MOS entered the bedroom and dragged her into the living room.

The case to be presented involves the details surrounding the death of Mrs. Ursuline Alfred while under the care of the Hollywood Medical Center, now named Memorial Regional Hospital South, on May 18, 1996. Mrs. Ursuline was a forty-five year old female believed to have suffered a seizure in her home at approximately 1930. She was found on the floor by her husband, after shouting “My head”, in a coma-like state. Mr. Alfred activated EMS and the patient was transported to HMC immediately. While in transport, Mrs. Alfred recovered and EMS recorded “near normal” vitals at 0800 per the District Court of Appeals of the State of Florida (2012).

CCIB received SOC 341 involving resident Camille Griffith (DOB: 03/09/84). Resident left a message with North Los Angeles County Regional Center (NLACRC) supervisor stating that her roommate, Patricia raped her. The resident later stated that it was consentual and then changed her statement to not consentual. Last week, the resident stated that she engaged in consentual sex. The story as reported by resident is that she slept with her roommate 's boyfriend.

When I arrived on shift we were advised during pass on that inmate Ellery, Danielle Jean (A-13367) was upset due to multiple inmates being showered out and placed in her cell (Cell 1). Her along with inmate Roskey, Skylar Lynne (A-14203) antagonized and bullied both inmate Knight and Robertson until they were at their breaking point. It got to the point where inmate Belanger tried to intervene and got in a heated argument with Roskey, which almost boiled over turning into a physical altercation between the two. Ever since inmate Roskey was placed with Ellery it created a cancer within the cell with the two feeding off of each other. As ordered by Sgt.

On 6/19/16 at 2341 hours my partner, Officer Acosta #0044, and I were working patrol assigned to unit 3A22. We responded to a radio call of “shots fired” in the area of 101 W. Washington Bl. Prior to our arrival, Sergeant Gomez #3196 arrived on scene and advised that there was a female, victim of a possible gunshot in front of 101 W. While I was standing in the atrium around 1115 I noticed the patient I just escorted out was walking into the heart lobby with a woman Ellen Laney. as they entered the atrium she was yelling that he just hit her. So I pursed Thompson through the breeze way to the parking garage stopping him and asking him if he had hit that women back there he told me no I was just standing to close to her I was just asking for a ride home. I proceeded to escort Thompson off campus and stayed in the heart parking lot making sure he did not come back.

It’s most definitely challenging working with mental health inmates and they require a lot of assistance, but if something was going on to cause harm to Ashley then this Officer should of called for back up to enter the cell. Specific intervention that should of taken place was that the Officer should called for back up, instead of using inappropriate behaviour in front of Ashley. That wasn’t the appropriate way to handle the situation especially involving someone who is at harm of either themselves or surrounding

Before I even step foot in the door I could hear sobbing and the distress in a woman 's voice. I carried in the necessary equipment, the cops were actively doing CPR as the advanced life support provider (ALS) was looking for IV access. As EMS personnel we took over for the cops, we placed a LUCAS device on him, it 's a device that does compressions for you. Then, we continued ventilating the patient and placed him on a backboard in preparation to transport and move the patient.

While in the front yard of the residence a Hispanic female was screaming hysterically about Bustos assaulting her. I identified the female as Adriana Alvarez (TX-ID # 10496407), Alvarez stated, “Bustos assaulted me by trying to drive away with me on the truck I am pregnant”. I attempted to calm Alvarez down by reassuring her that she is alright I had EMS dispatched to the residence to check out Alvarez. Alvarez was cleared by EMS

Mr. Brown is a 35 year old male who presented to the ED via LEO following a standoff. His wife took out IVC paperwork out on Mr. Brown, which she reported he has been threatening suicide. Per documentation he told officers that if they come near him he'll take one of them with him. Before the assessment Officer Hurley was spoken with. He reports tonight they found Mr. Brown in his truck and asked him to get out and come with them.

On 9/11/17 Joseph Gomez, Wellness Coach (WC) made a visit to the home of Mrs. Sergia De Jesus Espinal (Tenant) for the purpose of picking up her monthly rent money order and following up on her wellness goal to maintain her apartment clean and in sanitary condition and to address any issues she may be experiencing. Tenant has been attending her therapy and day program. Tenant was dressed with a drooping dirty house robe and appeared unkempt. Tenant appeared to be at her baseline. Tenant states she is doing fine and is content with her new futon bed.

How do you ensure your safety while being panicked and scared for your life? Is this considered a traumatic event? In order to answer these types questions we will analyze a real life incident that took place in 2007 involving long time criminal Jack Fitzmorris and victim Karen Zappitelli. On an April afternoon, Fitzmorris escaped from a hospital after overpowering a guard, stealing a uniform and revolver while holding some nurses and guards hostage, car jacked an individual, robbed two banks (Suspect in hostage case draws 35 years, 2008), and ultimately ended his spree when he broke into an accounting business and took the office manager Karen Zappitelli hostage (Associated Press, 2011).
