Hannah is a great asset to the Housing for Health program. Hannah came into the organization ready to work and learn. Hannah started the position as the clinical case manager and jumped right into dealing with clients. This had both a positive and negative effect on Hannah. The positive aspect of doing this was that Hannah was able to connect with her clients and was able to help clients with crises. The negative aspect of doing this was that Hannah was not fully prepared to handle the situations she encountered. So with Hannah jumping into to the work of helping clients, she was not fully able to handle all situations and crises immediately. Hannah had to do the work and learn the job at the same time. Hannah starts each day client centered and wanting to truly help change the lives of her clients. Hannah makes the effort to help her clients get through crises, solve life problems, avoid issues, and most of all ensure she is ending homelessness for clients. As this is a strength of Hannah’s, there is also an area of growth attached to it. Hannah needs to work on boundaries with her clients. She has not done anything that is alarming as a case manager but she needs to find the balance of case management. Hannah needs to work on not being to close to clients and at the same time not to far away. The goal for Hannah would to be to create that even balance between amongst her entire case load. As there are some clients she has a great rapport with and spends a lot of time with them. There are also clients she hasn’t built a strong rapport with and do not see them that often. Hannah needs to work on preparing interventions for her clients before she starts services, as sometime Hannah can get caught up in the client’s crises and forget to critically think as the client case manager. Hannah needs to focus on ensuring her clients become self sufficient and are not dependent on case
Are met by professionals, because due to their illness they are experiencing changes and therefore the relevant staff will be able to help with the specific care that they need for their specific needs. This will help professionals to be able to support the client in the best possible way that will benefit them overall when receiving their care

As I read the case study of Almeada and baby Anne, I was inspired by her case manager Barbra LaRosa, she provided social care and became the “bridge” between Almeade and the systems. One function of bridging is to narrow the gap between the services being offered and the needs of the individuals who are receiving those services. (Woodside, M. R. (2015). I felt like Rachel felt comfortable discussing her problems with me but I could have done a better job at getting to know her a little more. The focus is more on the individual than the problem. The client is viewed as unique and their own way. As the practitioner you have to treat your client situation as its own. The practitioner serve as a help to assist the client whit their choices they want to make .The practitioner helps the client by eliminating what is in the way of the client reaching their goal.
By establishing a worker/client relationship, this will provide Laura with a secure base to operate from in the future. She will be able to confidently explore her historical, current, and future relationship with her mother knowing that she can receive comfort and reassurance from me, her social worker. Once she recognizes this secure base, I will assist her in discovering how she currently handles her relationship with her mother. During this relational discovery process with her mother, I will also allow her to explore her relationship with me, showing Laura how her previous ways of dealing with others could be positively changed through the change of her various internal behavioral models. Through this social worker and client relationship exploration, Laura will discover how her current perceptions of her mother are connected to expectations from their relationship when she was a child, providing her the opportunity to view the current relationship differently.
