Family Nurse Practitioner: A Case Study

From the Kaiser Permanente career website, I was delighted to learn about the RN I position at Kaiser Permanente Oakland Medical Center in the Medical Telemetry Department. Kaiser Permanente is a leader in our nation’s healthcare system and has the standard for promoting wellness and disease prevention in our communities. As a result, I am excited about the possibility of being part an organization that so deeply shares the same passion as I of providing holistic and compassionate health care. Texas is home to nearly 10,000 nurse practitioners. Nurse practitioners (NPs) in Texas do not have as many freedoms as NP 's in many other states. In fact, Texas falls at the lower end of the spectrum regarding the freedoms it offers nurse practitioners. House Bill 1885 would grant full practice authority in Texas to all advanced practice registered nurses (APRNs), including nurse practitioners (NPs). It would authorize APRNs to evaluate and diagnose patients; order and interpret diagnostic tests; and initiate and manage treatments; prescribing of medications, these are currently job descriptions of APRNs and NPs that they are already doing, however, it would move these items under the exclusive licensure authority of the Texas Board of Nursing.

Scope of Practice NP’s scope of practice can vary depending on which state they are licensed in. It is extremely important that the NP is aware of the laws and regulations that their state has implemented. Understanding the scope of practice allows you to understand what you can and cannot do as an NP. In Arizona, NP’s are board certified and are now required to take a national certifying exam (Buppert, 2015). NP’s in Arizona have an expanded scope of practice, they can assess, manage, diagnose, and prescribe medications to patients.

Nurse practitioners will be present at all outpatient locations functioning as primary care providers in family practice and other practice settings (pediatrics, geriatrics, acute care, and other specialties). CNMs will be providing midwifery care along with primary health care of women. Nurse-midwives in United States have demonstrated excellent results in their field of practice throughout the years, especially attending to underserved, uninsured, low-income women (Lindeke, Fagerlund, Avery, & Zwygart-Stauffacher, 2010). CNSs will serve the role of case managers and care coordinators mainly at the hospital leading discharge planning of those patients with complex health problems. CRNAs will provide their services at the hospital for the patients in the intensive care unit.

Nursing has become a very dynamic role as it is constantly changing and evolving. The number of people entering the nursing field continues to increase. The responsibilities nurses are held accountable for are very diverse and demanding. Mental health nursing is one of the many diverse fields a nurse must be educated in. A nurse must be competent in attending to a patient’s mental health as it is something that pertains to all patients regardless of race, origin, or religion.

Nurse practitioners play an important role in healthcare in every state. However, each state has specific regulations pertaining to state regulations that guide prescriptive practice related to educational requirements, advance practice licensure requirements, and prescriptive authority (specifically controlled substances) of nurse practitioners. This paper will analyze the governing regulations of nurse practitioners in three states with full practice authority, reduced practice authority, and restricted practice authority. It will compare and contrast Washington D.C., New York, and Georgia, which have similar educational and licensure requirements, but vastly different approaches to the scope of practice in their respective states.

My passion for nursing practice defies description; leading to the reason I am opting to pursue a higher level of education in nursing. The George Washington University School of nursing has one of the highly ranked master’s programs in the country, which offers a competitive curriculum and highly defined leadership skills that integrates technology into learning. It is therefore my desire to pursue an advance education at this facility because it prepares practitioners to become great clinicians ready to solve real-world clinical problems. Moreover, the opportunity to pursue a degree in this facility will afford me the ability to be a competent and highly efficient family nurse practitioner; that will serve the community and mostly the underserved. I am the last child in a family of eight that has aspired to pursue my education at the graduate level.

As a bedside nurse, I believe that it was significant that Lewis’s crisis developed over the weekend. Typically staffing levels are lower on the weekends and staffed with less experienced staff. I currently work at a teaching hospital and this is true to some extent at my hospital. However, we have a rapid response team that operates inpatient 24/7. The team consists of a registered nurse and a respiratory therapist.

In My role as a DNP practice scholar, I will address the critical skills essential for translating the evidence-based care into practice with the aim of improving systems of care within the clinical setting (Vincent, Johnson, Velasquez, & Rigney, 2010). Besides, the practice competencies and research I will use the nursing skills to measure the outcomes of the affected population as well as communities. I will apply both clinical investigation and theory with affected individuals and families and implement nursing strategies to improve patient outcomes in nursing homes. I will achieve this by focusing on evidence-based practice, evaluation of results from nursing practices, and focusing on quality improvement (Vincent et al., 2010). As a DNP practice scholar, I will conduct research as a means of creating new knowledge within the nursing home clinical settings.

I just completed my Bachelor of Science in Nursing and look forward to exploring nursing theory, evidence-based practice and leadership even further in the advance practice curriculum. I am pursuing family nurse practitioner because it will allow me to be more versatile in my nursing skills and serve a larger patient population. There has been a focus on primary care medicine since the Affordable Care Act; resulting in a larger need for nurse practitioners required to care for this patient population. I have been granted an opportunity to advance my career and I am more than ready to take on the added responsibilities and handle treatment plans autonomously, as well as learning additional skills such as diagnosing and prescribing medications. By obtaining my graduate level education my clinical judgment would be enhanced to improve safety, quality of care and patient outcomes substantially.

Nurses are critical for promoting health in the society. The profession is highly flexible, since they specialize in diverse operations in the medical field. Registered nurses, for instance, are responsible for the administration of medicine and inoculations to patients (American Nurses ' Association, 2000). Additionally, these professionals observe, record, and enlighten doctors of any changes in a patient’s health. Nurses interpret and evaluate diagnostic examinations to determine an individual’s condition, as well as making the necessary adjustments in patient treatment plans on their health progress.

This is a personal statement for my application to the Family Nurse Practitioner Concentration in the Ball State School of Nursing. My personal statement provides details on my personal background and the unique impact that Family Nurse Practitioners have had on my life. I describe my undergraduate studies and how I firsthand gained interest as well as insight on the skills of a Family Nurse Practitioner. I go into detail about my aspiration to be a Family Nurse Practitioner to directly affect the patient care of a variety of patient populations. A homecare nurse is in the process of making her visits. Her next stop is a new family, the Solitano's from the movie, Silver Lining’s Playbook (Russell, 2012). Before arriving, she knows very little about the family. She knows that there are three members, Pat Sr., Dolores, and Pat.
