I. INTRODUCTION
1.1 DRUG USE EVALUATION
Drug use evaluation (DUE) can be defined as an authorized, structured, ongoing review of prescribing, dispensing and use of drugs. DUE encompasses a medication review against predetermined criteria that results in changes to medication therapy when these criteria are not fulfill. It includes a comprehensive evaluation of patients' prescription and medication data before, during and after dispensing inorder to ensure appropriate medication decision-making and leads to better patient results. As a quality assurance measure, DUE programs aims corrective action, prescriber feedback and further evaluations.
DUE is classified in three categories:
Prospective - evaluation of a patient's drug therapy

DUE programs play a important role in helping managed health care systems to understand, interpret, review and improve the pattern of prescribing, administration and usage of medications. Employers and health plans find DUE programs effective since the outcomes are used to foster more effective use of scarce health care resources.
Pharmacists play a important role in this process because of their skill in the area of drug therapy management. DUE affords the managed care pharmacist the chance to identify trends in prescribing within community of patients whether by pathological-state such as those with asthma, diabetes or high blood pressure, or by medication-specific criteria.
Pharmacists can then, in association with prescribers and other members of the health care team, start the action to improve medication therapy for patients.
1.1.2 BACKGROUND
DUE is an concurrent, systematic process developed to maintain the appropriate and effective use of drugs. It includes a comprehensive evaluation of a patient’s drugs and health history before, during, and after dispensing in order to attempt to get accurate therapeutic decision-making and better patient

Prospective DUE: Prospective review includes reviewing a patient's planned medication therapy before a medication is dispensed. This process allows the pharmacist to identify and rectify problems before the patient has obtain the drugs. Pharmacists periodically perform prospective evaluation in their daily practice by reviewing a prescription medications dosage and directions while evaluating patient information for possible drug interactions. When part of an online claims adjudication process, prospective DUE sometimes relies on computerized algorithms to perform key checks involving drug interactions, duplications or contraindications with the patient’s disease state or
Healthcare professionals must talk to their patients about possible side-effects of drugs they are taking and make sure they understand what can happen. In doing so, patients may start to understand why something is happening to them and it is a normal side-effect, which can not only lead to trust from the patients to providers, but can lead to the passing of knowledge from one to another which may prevent future
MTM is used to describe the broad range of health care services provided by pharmacists. These services include comprehensive medication reviews, medication reconciliation, drug use review, the ordering and review of lab tests, immunizations, drug dosage adjustments, and identification of gaps in care. Integrated systems of care, such as accountable care organizations (ACOs), already view MTM as essential to care delivery and to meeting ACO quality and cost targets. Such organizations also are heavily invested in HIT, including e-prescribing and EHRs. MTM can improve medication adherence and patient outcomes among patients suffering from chronic diseases, thus cutting costs and improving the quality of care and patient
The assessment that I would use for this client would be the Drug Use Screening Inventory-Revised. This assessment consists of true and false that address the client’s substance abuse, physical health, mental illnesses, family relationships, other relationships, education, vocational status, social skills, hobbies, or negative behaviors (Brooks & McHenry, 2015, p. 89). By completing this assessment, the therapist can obtain more information about Sara’s history, relationships, mental health, and behaviors.
Legislation P3- Explain relevant sections of key legislation and associated guidelines with regard to the administration of medicines. M1- Discuss how organisational policies and procedures are influenced by legislation and guidelines with regard to the administration of medicines. D1- Evaluate the effect of legislation and guidelines on the administration of medicines. In this assignment I am going to be explaining what different types of legislations and guidelines are in place when it comes to handling medicines in a health and social care setting.
Preventable medical mistakes cause approximately 200,000 deaths around the United States each year. (1) More than 1,000,000 Americans are negatively impacted by medication errors each year caused by inadvertent mistakes in the prescription filling process. With 4 out of 5 adults taking at least 1 medication daily and 1 out of 4 adults taking 5 or more medications daily nationwide, errors like these cost healthcare industry billions of dollars per year. Health information technology were developed to transform healthcare services, the way they are provided and compensated. Electronic prescribing (e-prescribing) becomes an internal part of that transformation process, which can be confirmed from annual Surescripts’ National Progress Report.
Pharmacists are in a unique position to help. They have the ability and knowledge to implement programs as part of their daily practice to ensure that patients are adherent to their medications. As the medication experts, pharmacists should lead the way to improving medication adherence and providing optimal patient care. The provisions of this bill do not pose a mandate but where appropriate would require the proration of prescriptions, related cost sharing, and dispensing costs in order to conform the patient to one monthly refill that occurs on the same date each month.
(Rissmann et al., 2012). Hence, considering the aspect of patient, NMC has picked up a cue as it is used by pharmacists while helping them in advising people in order to buy medications. Thus, this aspect is not strong enough to go into isolation as if it is combined with the charge, which ensures that medication history is taken along with OTC, herbal, POMs, allergies since patients need to explain that these aspects need to explain that which aspect is essential. Hence, assessment of holistic needs of patient looks for the determinants of health. However, patients frequently omit to explain regarding the OTC preps plus the herbal needs, which also needs to explain the usage of contraindicated drugs as it all explains the illegal needs of the
What outcomes will be analyzed and how they will be analyzed? Although the veterans are asked their full name and last four of their social security, the scanning provides the added comfort of providing the security or safety for everyone involved. The outcome of this research will show data that will be analyzed pre and post implementation of the BMCA system, which the approach is to show a significant change in the medication error rate. The outcome will be based on pre and post implementation of the barcode medication system by measuring the medication error rate.
There is a lack of support from physicians as a separate medication ordering system will be used, creating an increased in workload and reduction in patient interaction time (Park et al.,
In this case the concurrent review was chosen. As discussed previously in the assignment it was decided that drug kardex documentation would be audited. A drug kardex, also known as drug prescription or drug script is defined by the World Health Organisation (2002) as ‘’an instruction from the prescriber to the dispenser’’. In this instance the prescriber will be identified as any doctor in the hospital setting with prescriptive authority and the dispenser can be identified as any registered general nurse.
How Oratect 6-Panel Saliva Drug Test Kit Can Help Your Company Drug abuse remains a major concern in any country. Despite strict measures implemented by the government in ensuring the whole community to be safe, drug manufacturers still find ways on how they can continue with their business and spread them out to people. It is indeed alarming how drugs affect the lives of so many. Studies show that there are drug users practically in every industry. With this fact, it is only essential that the safety of everyone should be prioritized.
Safe medication administration is a big aspect of nursing care, because if medications aren’t given safely, then it can lead to some serious adverse effects to the patients. There are many things that can go wrong, and that’s why nurses have to be very careful when handling and giving medications. Nurses can make mistakes, and give the wrong med, give it to the wrong person, or even give too much or too little of the drug. Careful medication administration can lead to not making big mistakes that can lead to hurting others. “Medication Administration is a complex multistep process that encompasses prescribing, transcribing, dispensing, and administering drugs and monitoring patient response.”
Medication Adherence Reflective Writing Shaymous Juhnke As a P1 student in SDSU’s pharmacy program one of the activities required to prepare us for real world pharmacy practice would be to take part in a medication adherence simulation. The goal of this activity is to put ourselves in the patients shoes to get an idea about how patients adhere to their regimens in the real world. Through this activity I have learned that it is not always easy to take medications at the right times.
7.2% of 8th graders, 17.6% 10th Graders and 22.6% of 12th graders have used marijuana. Student drug use is a big problem, but is it worth the random drug tests. tHere are many different opinions on this topic, but they may not know all the facts. There are good and bad parts to random drug testing students. There are a few laws in place and some cases have been filed.
Topic: Need and importance of clinical pharmacists in healthcare system Clinical pharmacy is a health science discipline in which pharmacists provide patient care that optimizes medication therapy and promotes health, and disease prevention, according to American College of Clinical Pharmacy (ACCP). Patient-oriented care requires specialized knowledge, skill and experience to improve quality of life of patient. The development of clinical pharmacy began at the University of Michigan in the early 1960s. Instead of drug-oriented pharmacy, patient-oriented concept was initiated within short period of time.
