In his complaint, Mr. Filasky states he contacted Verizon to inquire about a very important number he was interested in. The representative he spoke with advised him he would call him back after looking into the numbers availability. Mr. Filasky received two package a day later which he did not open. He was waiting for the representative to call him back. After waiting for two weeks and not receiving a response Mr. Filasky called back and was advised to return the packages to a store. The store was not able to assist him as well. Since then he has been referred to the Fraud and Retention department who have also not been able to assist him with the return because it is past the 14 day return period. On 09/06/16 I contacted Robert Beck Jr., with Apple Tree Service, at 3603 Roderweis Road in reference to a criminal mischief call. Mr. Beck said sometime over the nighttime hours someone damaged two of Apple Tree Services trucks that were parked in the parking area of the above address. Mr. Beck said one truck had a hasp cut off of a cargo box and another truck a door bent, from being pried open, on a cargo box. Mr. Beck said there did not appear to be any missing items from either truck. Mr. Beck could not provide any suspect information and estimated the damage to the two vehicles at $200 per vehicle.
S/S Folwaczny contacted Non- Emergency 911 concerning two black sedans parked on the premises in front of OPS 1 at 12490 Sunrise Valley Drive. S/S Folwaczny spoke to Dispatch Operator Dang concerning the vehicles, After running the VIN number, D/O Dang was able to confirm that the vehicles were not stolen and that any further action pertaining to the vehicles would have to take place Sprint Security.
on 08/18/2017 friday i officer bell was dispatched to the station in regards to a walk in. upon my arrival i made contact with a donna campeau. campeau stated that on 08/17/17 Thursday she had tried to use her debit card at Dodges gas station and it was declined. she later found out that $293.50 had been used to pay a sprint wireless phone bill.

The plaintiff lives in Michigan with her husband and children and was undergoing marital difficulties. She had intended to file for divorce. December 6, 1963 the defendant appeared at her home and introduced himself as “Dr. Wolodzko.” He had never met the plaintiff or husband prior to the visit and he stated that the husband had called him. The plaintiff testified that the defendant told her that he was there to just ask about the husband’s back and that he never told her he was a psychiatrist.
Berthoud police officer, Jeremy Yachik, was accused of abusing his 15-year old daughter in the year of 2014. An email received by the Loveland police department included video footage of a man, later identified as Yachik and his daughter, hitting and kicking a young female in 2012 which led to an investigation and later, an arrest. Loveland police investigated the allegations instead of the Berthoud Police department due to Yachik’s ex-fiance claiming that she had emailed the video to Berthoud Police Chief Glen Johnson but Johnson had never responded, thus causing her to continue trying to get help for Yachik’s daughter. The abuse Yachik’s daughter accused was inflicted upon her included being bound with plastic zip ties, being force-fed hot sauce, as well as being choked.

On December 15, 2016, local police found a gang member of the royals, identified as 16-year-old Andy Larsen, lying on the sidewalk near Woodbine and Dixon Road unconscious. After further investigation, police found no vitals and Larsen was declared dead at the scene. On the above date I responded to Walmart, reference a trespasser. Dispatch advised me that Walmart LP manager, William "Bill" Dorkowski, was requested that a male subject be trespassed. Matthew Wawrykiewycz was arrested by, York region Police, on February 9, at his home in Aurora, after police confiscated two kilograms of cocaine and $100,000 of cash. Police began the investigation in September and later identified Wawrykiewycz as the suspect of the sale of illegal substances.
