Focus: Ms. Smalls (MHP), Ms. Givens (MHS) and Antazia review the draft treatment plan and progress school behaviors. Intervention: MHP, MHS and Antazia review the draft treatment plan. MHP answered MHS and Antazia questions about the goals and interventions. MHP answered Antazia questions about kinship care and updates from her DSS worker. MHP progress and role-play different ways to communicate with school officials.

Circumstance: Ayden will maintain contact with medical team monthly. Ms. Smalls (MHP) and Mrs. Wigfall (MHS) discuss Ayden’s recent medical appointments and therapy. Action: MHS report Ayden will start physical therapy at an outside clinic. MHP and MHS discuss Ayden receiving all therapy at the same clinic to reduce several therapy appointments during the week. MHP and MHS review reports given since last week.

worked for the Children’s Center of Cicero Berwyn as a part-time Home visitor for six months from September 2016 thru March 3rd, 2017. She was a good employee who had passion to work with children and families. She created lesson plans for five children and families, prepared developmentally appropriate activities, observed children, provided resources and referrals to families in the communities. Her doctor had instructed accommodations and those were in place for her. Those accommodations included not to sit or stand for prolonged periods of time and not to lift more than 10 pounds.

On 3/29/2016 I spoke with Leahanne Garcia at the operation. Leahanne stated she has been employed at the operation since May 2015 as a Teacher assistant. They cater for all children no matter if living with disabilities or lacking motor skills. Usually the mobile classroom comes equipped with special features for those who have a disability, for example, the Healthy Harold mobile classrooms are wheel chair accessible and offer informative services for children that could be deaf or blind.

The target child of this observation is Sofi. She attends full day childcare at The Little Skool-House International @ Tampines Junction. Sofi has been attending the preschool since she was two years old. She also has a younger sister who is currently two years old and attends the same preschool.

While Jose and Jorge are identical twins, their lives could not have been any more different. They have different health determinants to their unfortunate diseases that differ from each other. The case study of Jorge and Jose emphasizes the idea that your zip code determines your health more than your genetic code. Just a quick reminder, Jose is the twin who grew up in a largely improvised family, while Jorge is the twin who grew up in an upper-middle-class family. Since both brothers have asthma it can be assumed that the brothers have asthma in their genetics, however, needed to be triggered.

Self-determination. As Alvin move towards adulthood, he should be able to demonstrate a certain level of self-determined behaviors. He is expected to better manage his self-regulatory behavior, be increasingly independent in his daily life skills and be involved in his personal future planning. Essentially, self-determination provides a form of empowerment and having the abilities to steer a person’s own life, resulting in a more positive quality of life outcome. In promoting self-determination in my classroom teachings, Alvin will be more engaged in his schoolwork, improve in his work performance and behavioral outcomes.
