Claire Conroy was an 84-year-old patient in a nursing home. She suffered from severe organic brain syndrome, necrotic decubitus ulcers, urinary tract infection, arteriosclerotic heart disease, hypertension, and diabetes mellitus. She was unable to speak or move, but sometimes able to follow people with her eyes. Nurses were not able to feed her by hand, so a nasogastric tube was inserted to be able to provide nutrition and give all her medications and fluid this route. Her only surviving relative, her nephew and legal guardian, requested the Superior Court of Essex County, New Jersey, to allow the nasogastric tube to be removed and thus allow Claire Conroy to die.

Resident E.V. has demonstrated the ability to cope by displaying effective coping patterns. She has the desire to become both healthier and live a healthier life style. Her goal is to gain recovery so she can walk on her own again and eventually move back into her house to help take care of her husband, whom has dementia. She is willing to modify her lifestyle accordingly if she is unable to gain complete recovery. However, she would like to be as independent as possible.

Sara – Mel’s wife and caregiver– had deteriorated in physical and emotional health, until she discovered Joe’s Club. She could go and attend to her needs and get a well-deserved break at least a few times a week, like many other caregivers. I began to now understand the crucial role ADCs played in their lives and grew passionate about it. Outside of Joe’s Club, the awareness about ADCs barely existed and so many caregivers did not seek help that they deserved. To promote awareness, I used my Girl Scout Gold Award project to develop a community-based survey which was designed to assess the awareness, benefits, and challenges about the use of ADCs.

Supplementing external aids to promote problem-solving with residual capabilities is a crucial part of this frame of reference. Useful approaches within this frame of reference for the case of Sarah would be compensatory approach and adaptive skills approach. Compensatory approach is widely used to compensate for dysfunction in mobility, self-maintenance and domestic activities, and methods of compensation may include the supply of assistive equipment, modification of the environment and organization of social assistance. The adaptive skills approach aims at helping the person to adapt his/her existing skills to master problems and cope independently in various circumstances.

As the assessment begins for Mrs. Adams there are many things that need to be addressed. Educating the patient on active participation in her healing will be of extreme importance. The interventions are only as good as the patient’s willingness to comply. We would need to begin with gaining control of the cleanliness of the apartment and that of Mrs. Adams. Opening the windows and allowing the sunlight to enter is important along with attempts at fixing the air conditioner.
